Upon re-entering Earth's atmosphere, Columbia breaks apart over Central Texas, claiming the lives of commander Rick Husband, pilot William McCool, flight engineer Kalpana Chawla, mission specialists Laurel Clark, Michael Anderson and David Brown, and Israel's first astronaut Ilan Ramon. Debris recovery operations begin immediately. Shuttles Discovery, Atlantis and Endeavour are grounded indefinitely. The next day, the Columbia Accident Investigation Board, or CAIB, headed by retired Navy admiral Harold Gehman, is appointed to determine the cause of the accident.
